Political rivals have strongly criticized First Minister John Swinney for refusing to specify how many medical positions will be eliminated during upcoming public sector cutbacks. The trade union Unite strongly condemned the consolidation strategy, describing the health board reduction as a “slash and burn programme for the NHS”. Challenged repeatedly during an interview on BBC Scotland’s The Sunday Show, the Scottish leader acknowledged that overall civil employment would decline, yet he insisted, “I can’t put a specific figure on that today and I simply wouldn’t be giving a comprehensive answer if that was the case.” He argued that ministers must systematically execute “a programme of reform to make sure that we put people right at the very centre of the delivery of public services so they can see the enhancement in the provision of those services in their localities.” Expressing serious concern over the lack of clarity, Scottish Labour’s acting leader Jackie Baillie said “It’s hard to tell whether John Swinney is being evasive or simply has no idea what he is going to do, but either way it’s not good enough. People’s jobs are on the line and our public services could be plunged into chaos – Scotland deserves to know what lies ahead. The SNP must come clean about what it is planning, how many jobs are at risk and what it will mean for our public services.”

The controversial restructuring, announced during Swinney’s recent legislative roadmap, aims to reduce Scotland’s current 14 regional health administrative bodies down to just two. Characterized by the First Minister as the “most significant structural reform in the NHS in the era of devolution”, preliminary design work commenced last November. Under this proposed framework, administrative operations would split into eastern and western zones overseen by the leaders of NHS Lothian and NHS Greater Glasgow and Clyde. While local municipal councils might eventually assume responsibility for healthcare across island communities, official decisions regarding those regions have not been finalized.

Amid anxiety that the consolidation could force patients to make grueling journeys for treatment—such as expectant mothers in the far north traveling 100 miles along the A9 to give birth in Inverness—Swinney suggested that it would “not be my intention” to increase transit distances. Emphasizing that clinical safety takes precedence over financial savings, he pointed to maternity provisions in Caithness as an example of safety-led decision-making. He maintained that the central goal of eliminating administrative partitions is to guarantee “equity of access” for every citizen regardless of geographic location. Swinney added: “The example in Caithness is an issue about the delivery of maternity services in Caithness. And that’s driven not by efficiencies, it’s not by costs, it’s delivered by the question of patient safety.” Clarifying his vision for community-based healthcare, he remarked: “What I want to make
